Great news for individuals on DDA’s No Paid Services (NPS) caseload. DDA has just made requesting services a lot easier with its new online Service Information & Request form.
If you or your family member have been determined eligible for DDA and would like more information about what’s available, or would like to request specific services, fill out the online form and someone from your regional office will get back to you.
